FWD dialup don't use any NAT handling as far as I'm aware. Try configuring your device with a STUN setting so that it can work out it's public IP address.

Yes, it registers properly if the MySIPSwitch account name is the same as that of the pFingo account. pfingo are matching on the name in the Contact header. I've been able to accomodate this for outgoing calls but it's currrently not possible to easily do the same for registrations.
